In order to complete your registration to AntetokounBros Academy you should send us at [email protected] the following:
Until October 20, 2020:
- Acceptance of the new terms & conditions of participation as the try outs procedure has been removed. See the new terms & conditions of participation HERE
- Image Release Form find HERE. You must send it to us completed and signed
The email you will send must contain the acceptance of the terms of participation (I ACCEPT THE TERMS & CONDITIONS OF PARTICIPATION) together with the Image Release Form document attached and in the subject of the email you must indicate the name of the athlete.
Medical Documents:
For the required medical documents, HEALTH CARD & MEDICAL CERTIFICATION an appointment will be scheduled with a doctor of the Onassis Cardiac Surgery Center for the examination of the athletes. Your presence at the doctor’s appointment is mandatory. In case of absence and not bring us the medical documents the participation in the Academy will be canceled.You will be informed about the date and time of the appointment in the next update.
- Medical certification find HERE. The Athlete’s Medical History document mentioned in the Medical Certification is confidential and should not be sent to us. You will find it HERE
- Athlete’s Health Card can be found HERE. The card must have a recent photo of the athlete’s face and be signed and stamped by a cardiologist
