For athletes to participate in the educational programs, parental consent is required for their transportation by the program’s bus. Please sign the consent form, which you can find here, and send it (with a photo or a scanned copy) no later than Wednesday December 6 to the academy’s email at [email protected].
Please fill out the form you will find here for drug allergies or if the athlete receives any specific medication.
